Which drug is most dangerous for the heart?

The answer depends on what type of “drug” you mean (prescription medicine, recreational drug, or performance-enhancing substances), because different drugs can harm the heart in different ways.

Some substances considered high risk for the heart include:

  • Cocaine — can sharply increase heart rate and blood pressure and is linked to heart attacks, dangerous heart rhythms, and stroke, even in some people without known heart disease.
  • Methamphetamine and other strong stimulants — can cause severe strain on the heart, abnormal rhythms, heart muscle damage, and heart failure.
  • Opioids (especially in overdose) — can slow breathing, reduce oxygen supply, and lead to fatal heart and circulation problems.
  • Anabolic steroids used for muscle enhancement — can raise blood pressure, worsen cholesterol levels, and increase risk of heart disease.
  • Some prescription drugs (depending on the person, dose, and medical condition) can also affect heart rhythm or blood pressure, including some medications that prolong the QT interval and increase the risk of dangerous arrhythmias.

If you mean a specific drug name, tell me which one and I can explain its heart risks. If you mean a medication you are taking, include the name, dose, and why it was prescribed.
