Nail Lines: What They Can Reveal About Your Health

Lines on your nails can sometimes be completely normal, but in some cases they can reflect an underlying health condition. The appearance of the lines is important.

Vertical (lengthwise) ridges

  • Run from the cuticle to the tip of the nail.
  • Very common, especially with aging.
  • Usually harmless if there are no other nail changes.

Horizontal lines (Beau’s lines)

  • Run across the nail.
  • Can occur after a serious illness, high fever, major surgery, significant stress, or an injury to the nail.
  • They form when nail growth temporarily slows or stops.

Dark vertical streaks

  • May result from minor injury or increased pigmentation.
  • However, a new dark streak—especially if it’s on only one nail, is widening, or extends into the surrounding skin—should be evaluated promptly because it can rarely be a sign of Subungual melanoma.

White lines

  • Small white spots are usually caused by minor trauma to the nail.
  • White lines that span the entire nail or affect multiple nails may occasionally be associated with certain medical conditions or low blood protein, though there are many possible causes.

Spoon-shaped nails

  • Nails that curve upward (called Koilonychia) can be associated with iron deficiency, though they can also have other causes.

Pitting

  • Tiny dents or pits in the nail surface are commonly associated with conditions such as Nail psoriasis and can also occur with some other skin disorders.

When to seek medical evaluation

Consider seeing a healthcare professional if you notice:

  • A new dark streak on one nail.
  • Horizontal lines affecting several nails after an unexplained illness.
  • Nails becoming thick, brittle, painful, or separating from the nail bed.
  • Nail changes accompanied by fatigue, weight loss, swelling, persistent rash, or other concerning symptoms.

Most nail lines are not a sign of serious disease, especially simple vertical ridges that develop gradually with age. However, persistent or unusual changes—particularly new dark streaks or changes affecting a single nail—are worth having assessed.
