How to achieve a deep piriformis stretch to eliminate back, hip, gluteal, and leg pain

The title “How to achieve a deep piriformis stretch to eliminate back, hip, gluteal, and leg pain” makes a strong promise, but a piriformis stretch does not eliminate all types of back, hip, or leg pain. It may help some people, especially when tight or irritated hip muscles contribute to discomfort.

What is the piriformis?

The piriformis is a small muscle located deep in the buttock. It helps with hip rotation and stability. In some people, irritation around this area may contribute to buttock pain or symptoms that travel down the leg.

A gentle piriformis (figure-4) stretch

  1. Lie on your back with both knees bent.
  2. Place your right ankle over your left knee, forming a “4” shape.
  3. Hold the back of your left thigh and gently pull it toward your chest.
  4. You should feel a stretch in the right buttock or outer hip.
  5. Hold for 20–30 seconds and repeat 2–4 times per side.

Other helpful approaches

  • Gentle walking: Keeps the hips and back moving without excessive strain.
  • Hip and glute strengthening: Stronger supporting muscles can reduce stress on joints and the lower back.
  • Regular movement breaks: Avoid sitting for long periods.
  • Heat or gentle massage: May help relax tight muscles for some people.

Avoid forcing a “deep” stretch

A stretch should feel like mild tension, not sharp pain, numbness, or worsening symptoms. Pushing too hard can irritate muscles or nerves.

When to get medical advice

Back, hip, and leg pain can also come from conditions such as:

  • Disc problems in the spine
  • Arthritis
  • Hip joint problems
  • Nerve compression
  • Injuries

Seek medical care promptly for new leg weakness, loss of bladder or bowel control, numbness in the groin area, fever, or severe pain after an injury.

A physical therapist or healthcare professional can help determine whether the piriformis is actually the source of pain and recommend exercises suited to your condition.
