That Sharp Heel Pain in the Morning Could Point to an Underlying Problem

Sharp heel pain when you first get out of bed is most commonly caused by plantar fasciitis, but it’s not the only possibility. If the pain is persistent or severe, it’s worth having it evaluated.

Common causes include:

  1. Plantar fasciitis (the most common cause)
    • Sharp pain under the heel with the first steps in the morning or after sitting.
    • Often eases after walking a little, then may return after prolonged standing.
  2. Achilles tendinopathy
    • Pain at the back of the heel, especially with walking or climbing stairs.
  3. Heel spur
    • May occur with plantar fasciitis, though many heel spurs cause no symptoms.
  4. Stress fracture
    • More likely if the pain began after increased activity or doesn’t improve with rest.
  5. Nerve irritation
    • Burning, tingling, or numbness along with heel pain may suggest a nerve problem.

What you can try

  • Stretch your calf muscles and the bottom of your foot before getting out of bed.
  • Wear supportive shoes and avoid walking barefoot on hard floors.
  • Reduce activities that worsen the pain for a few days.
  • Apply ice for 15–20 minutes after activity.
  • If appropriate for you, an over-the-counter pain reliever may help.

When to seek medical care

See a healthcare professional if:

  • The pain lasts more than a few weeks despite self-care.
  • You can’t bear weight on the foot.
  • The heel is swollen, red, or warm.
  • You have fever, numbness, or significant weakness.
  • The pain started after a fall or other injury.

Morning heel pain is often treatable, but getting the correct diagnosis is important because the best treatment depends on the underlying cause.
