7 medications that cause severe dementia and profound brain damage

There are no medications proven to commonly cause “severe dementia and profound brain damage” in otherwise healthy people. However, some medications—especially with long-term use, high doses, older age, drug interactions, kidney/liver problems, or sensitivity—can cause confusion, delirium, memory problems, or cognitive impairment, and some are associated with a higher risk of dementia-like symptoms.

Examples of medication classes that can affect cognition include:

  1. Strong anticholinergic medications
    • Examples: some older antihistamines (such as diphenhydramine), certain bladder medications, and some antidepressants
    • Possible effects: confusion, memory problems, impaired attention; long-term heavy exposure has been associated with increased dementia risk in observational studies.
  2. Benzodiazepines
    • Examples: diazepam, lorazepam, alprazolam, clonazepam
    • Possible effects: sedation, slowed thinking, falls, memory impairment; long-term use is associated with cognitive concerns.
  3. Opioid pain medications
    • Examples: morphine, oxycodone, fentanyl
    • Possible effects: confusion, reduced alertness, delirium—especially in older adults or at high doses.
  4. Certain antipsychotic medications
    • Examples: haloperidol, quetiapine, risperidone
    • Possible effects: sedation and cognitive slowing; in people with dementia, some carry increased risks and require careful monitoring.
  5. Some anti-seizure medications
    • Examples: topiramate, phenobarbital, some older anticonvulsants
    • Possible effects: slowed thinking, concentration problems, memory difficulties.
  6. Corticosteroids (especially prolonged high doses)
    • Examples: prednisone, dexamethasone
    • Possible effects: mood changes, insomnia, agitation, and sometimes severe confusion or psychiatric symptoms.
  7. Certain chemotherapy and immunosuppressive drugs
    • Examples: some agents used in cancer or autoimmune treatment
    • Possible effects: “brain fog,” attention problems, and cognitive changes in some patients.

Important: A person who suddenly develops confusion, personality changes, memory loss, or a major decline in thinking may have a reversible cause (such as infection, medication effects, vitamin deficiency, thyroid problems, dehydration, or metabolic issues), not permanent dementia.
